Any inaccurate or incomplete declaration may lead to the suspension or end of our business relationship. By sending this form, the signatory confirms on behalf of the company that it will indemnify CosmLink B.V. for any loss, damage or expense — including legal and administrative costs, fines and penalties, and any other direct or indirect damage — and for any legal, administrative or criminal action arising from incorrect or inaccurate information given in this form.
How we use your information
CosmLink B.V. (Meester Marrestraat 80, 3032 SV Rotterdam, KVK 96852887) uses the information on this form to open and run your account, to apply the correct VAT treatment (including checking your VAT number in the EU VIES register), and to check that we may lawfully trade with you under EU sanctions law — which includes screening the names you give against the official EU, UN, US and UK sanctions lists. The legal grounds are the contract with you, our legal obligations, and our legitimate interest in knowing who we trade with.
Your answers and documents are encrypted in your browser before they are sent; only CosmLink B.V. can open them. The encrypted form is held for a few minutes by our hosting provider (Cloudflare) until we collect it. We keep the information for 7 years after our last transaction with you — the period Dutch law sets for business records — and then delete it. You may ask to see, correct or delete your personal data, or object to its use, through the contact details in our privacy policy, and you may complain to the Dutch Data Protection Authority (Autoriteit Persoonsgegevens).
